Wholesome Solutions Pty Ltd is seeking a dedicated NDIS Support Coordinator to join our team. This is a sub-contracted role, offering the opportunity to make a meaningful impact in the lives of individuals living with disabilities. As a Support Coordinator, you will play a vital part in navigating the NDIS system and ensuring our clients receive the tailored support they need to thrive.

What you'll be doing

• Coordinate and facilitate the implementation of NDIS plans for clients, ensuring their involvement every step of the way to meet their goals and needs
• Liaise with service providers, government agencies and other stakeholders to coordinate and monitor the delivery of supports
• Assist clients in understanding their NDIS plans and the available support services
• Advocate on behalf of clients to secure appropriate and effective support services
• Regularly review and update client NDIS plans and budgets to reflect changing needs and circumstances
• Maintain detailed records and documentation to support client progress and service delivery
• Collaborate with the broader Wholesome Solutions team to provide holistic care and support
• Flexible working arrangements - work from our centre in Caboolture with some WFH availability

What we're looking for

• Relevant diploma / tertiary qualification in a human services or allied health discipline is advantageous
• Min 6 months experience in a similar NDIS support coordination or case management role.
• Demonstrated understanding of the NDIS and experience navigating the scheme on behalf of clients
• Bring your current caseload - we welcome support coordinators who would like to bring along participants, whilst having the capacity to take on more as referrals come through.
• Excellent communication and interpersonal skills, with the ability to effectively liaise with a range of stakeholders
• Strong problem-solving and critical thinking skills to identify and address client needs
• Commitment to providing person-centred, empowering support to clients with diverse needs
• Willingness to travel within the local area to meet with clients as required
• Current driver's licence and access to reliable transportation
• Current NDISWC and Working with Children Check
• ABN as an Individual Sole Trader
