Join Allinto’s Ageing Services Team as a Care Coordinator!

We’re looking for an experienced Care Coordinators to join Allinto’s dynamic Ageing Services Team, where you can thrive in a collaborative, client-focused environment and showcase your care coordination expertise. This exciting three/six-month role is located at our Eight Mile Plains Office.

What You'll Do:

Deliver high-quality care services in line with organisational policies, Aged Care Quality Standards, and program guidelines

Coordinate client intake, assessments, and care planning, ensuring each plan is tailored to meet individual needs and goals

Monitor and adjust care plans based on changing circumstances, referring clients to Home Care Packages when necessary

Collaborate with clients, families, and service providers to create a seamless, high-quality service experience

Maintain accurate documentation, ensuring timely and compliant reporting

Promote a professional, respectful workplace culture, enhancing client experience and satisfaction

What We’re Looking For:

Demonstrated experience in a similar role, with a strong background in aged care, health, or community services (experience in a not-for-profit setting is a plus)

Advanced communication and interpersonal skills, with a strong focus on building relationships and ensuring client satisfaction

Understanding of the Aged Care Act, CHSP Guidelines, and Aged Care Quality Standards

Cert IV, Diploma, or higher in Community Services or a related field (desirable)

A passion for supporting seniors, particularly from culturally diverse backgrounds

Additional Requirements:

Working with Children Check and NDIS Workers Screening, or be willing to obtain

Right to work in Australia

Up-to-date First Aid and CPR certificates, or be willing to obtain

NDIS Workers Orientation Module – Quality, Safety & You
