Your Role as Care Partner

As the Care Partner you are responsible for delivering care management services to Support at Home customers to ensure they receive high-quality, person-centred care that meets their individual needs, goals, and preferences. You will work collaboratively with clinical care partners, customers, their registered supporters, carers, and other health professionals to develop and implement effective care plans, coordinate service delivery, and monitor outcomes .

You’ll empower individuals to live more independently, safely, and meaningfully by:

Work collaboratively with customers to identify their needs, goals, preferences, and existing supports in alignment with their Notice of Decision and support plan

Coordinate the delivery of services according to customers' approved services and care plans

Manage the quarterly budget to ensure no overspends occur and that services are delivered within funding parameters

Facilitate transitions in and out of different care settings and ensure continuity of care Monitoring, Review and Evaluation

Support customers to make informed decisions, including respecting their right to take risks

Manage the customers budgets to ensure that they do not exceed their allocated funding

Monitor and respond to changing needs and emerging risks

What We’re Looking For

Certificate III in Individual Support (Ageing), Certificate IV in Aged Care, or equivalent qualification

Minimum of 12 months experience in aged care, disability services, or community services sector

Knowledge of person-centred care approaches and ability to support customers in making informed decisions

Effective communication and interpersonal skills with the ability to work with customers, families, and health professionals

Ability to work both independently and as part of a team, managing time effectively to meet deadlines

Computer literacy including experience with client management systems and documentation

Physical capability to perform the role including sitting, standing, and traveling to customers' homes

A Valuable Addition, Yet Not a Must-Have!

Experience in broader healthcare or related settings

Additional qualifications in aged care or community services

Understanding of wellness and reablement approaches

Understanding of quality standards and continuous improvement processes

Essential Requirements

Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)

Relevant probity checks required by legislation and WMQ Limited policy.

Current Driver’s Licence and willingness to travel for work
