Services Coordinator - Position Description

Award: SCHADS Award 2010

Classification Level 4.1

ROLE PURPOSE

The primary responsibility of this role is coordinating services by Abundant Living Community Services support workers to deliver quality NDIS services to people with disabilities. This role is responsible for building a rapport with participants to understand their situation and coordinate service delivery, ensuring Support Workers are working with participants to meet the outcomes of each individual plan. Ensuring all operational requirements are met, such as matching and mentoring support workers with new participants. You will supervise and mentor a team of Support Workers. Manage the day-to-day operations of your team, ensuring participant needs are being met, and Support Workers are delivering support and reporting information according to procedures.

Liaise with the Operations Manager on operational and participant matters. Provide some direct support to participants where required and key liaison with support coordinators and other stakeholders. Collaboration to identify new service delivery opportunities to enhance the Abundant Living Community Services portfolio of NDIS and disability services. The Service Coordinator is a key position and helps achieve Abundant Living Community Services' mission and vision by genuinely working alongside people to achieve their aspirations.

Reporting Relationships

This role reports to the Operations Manager.

Direct reports are from team leaders, community nurses, drivers, and support staff.

Responsibilities and Key Duties

Manage a team of support service staff, community nurses, drivers and other support staff.

Oversee and coordinate direct support to participants within the framework of Abundant Living’s policy and procedures and within the context of the person's individual plan and or goals: -

Manage day-to-day employee matters such as performance, leave, and injuries with the support of the Operations Manager and corporate team. Ask for advice and support when needed.

Work with staff, participants, families, and other stakeholders to understand and resolve concerns. Escalate to the Operations Manager for advice and support as appropriate.

On-board new starters into your service/s in a way that provides them with the information they need to be successful.

Create and implement staff rosters and participant schedules, ensuring the best match for the participant.

Coordinate regular communication channels with support coordinators, employees, participants, and other key stakeholders.

Attend coordinator team meetings and undertake 1:1 discussions/supervisions with Support Workers.

Undertake direct support of participants, where required, including cancellations, periods of leave or staff sick leave.

Regularly review and refine support strategies to minimise risk and optimise best outcomes for individuals.

Maintain appropriate personal and professional boundaries.

Undertake all administrative tasks to ensure accurate records and quality of support, including risk assessments, regular goal reviews, and supportability data collection and sign-offs.

Assist the Operations Manager with the recruitment and orientation of new Community Mental Health Workers, as required.

Organisational Responsibilities

At times, being available to work in various Abundant Living programs when required.

Maintain accurate and timely records and collect data as directed by program guidelines and the supervisors.

Participate in performance appraisals, Abundant Living training, group supervision, team meetings and professional development activities.

Working as part of the team based on the ethos of collaboration, cooperation, respect, and mutual support.

Other duties as directed by Management.

Selection Criteria:

Qualifications

Qualification in community services, social work, or another relevant human services discipline (essential).

Experience working in the mental health field (desirable).

Current First Aid Certificate

Experience working with people with mental health issues (essential)

Experience in leading and mentoring people (desirable)

Experience in leading and facilitating community service programs (desirable)

Experience in rostering/ scheduling (desirable)

Experience in performance management (desirable)

Skills & Attributes

High-level written and verbal communication skills

Computing skills in the Microsoft Office package and experience in maintaining a client management system.

High level of interpersonal skills and the ability to network and work with a range of stakeholders.

Understanding of mental health/illness and associated issues.

Understanding of the NDIS plans and processes (desirable)

Additional Requirements Note:

A current and clear National Police Clearance kept in your HR file on Abundant Living’s employer database is a requirement of employment.

A valid unrestricted driver’s licence.
