We work with people, their families and communities.
About the Role
The purpose of the Senior Care Coordinator is to deliver Proveda’s Care Navigation model of service to older people, people with disability and people living with mental ill health. Working within a teaming framework, the Senior Care Coordinator will facilitate and provide high quality care navigation services.

About You
Relevant qualifications in community development, social work, psychology or other Allied Health.
Relevant experience in Care Coordination of Aged Care/NDIS services/Psychosocial Recovery Coaching.
Strong understanding of the current reform environment for one or more of the relevant systems of care and support (My Aged Care, NDIS etc.)
Confidence with facilitating peer to peer learning through a relatable presentation style in a group or individual setting with a focus on learning outcomes.
Detailed understanding of relevant national and state legislation and standards relevant to the operation of Proveda services.
Highly developed interpersonal skills including consultation, collaboration, openness, fairness, flexibility, liaison and problem-solving. Ability to listen effectively and with empathy.
Excellent written communication skills including experience in the preparation of high-level reports and case notes.
High level systems thinking and process improvement including the application of technology-based solutions.
Relevant experience in peer-to-peer mentoring will be highly regarded.
Proficiency with the use of relevant computer programs.
Current driver’s license.
Current NDIS Worker Screen.
Current Working With Children Check.
Current Criminal History Check.
