About the Role
As the Senior Discharge Coordinator, you will play a pivotal role in supporting the Neuroscience and Rehabilitation (NSR) program by providing expert clinical leadership and coordination for patients with complex care and discharge needs. Working closely with a multidisciplinary team, you will guide and facilitate timely, safe, and sustainable discharge planning, particularly for patients on the National Disability Insurance Scheme (NDIS) pathway or those facing significant discharge barriers. Your clinical expertise will ensure that patient transitions are managed holistically, with a focus on continuity of care and the best possible outcomes for each individual.
In this leadership position, you will act as CALHN’s subject matter expert on the NDIS and alternative discharge pathways, ensuring that the organisation maintains a consistent, informed, and collaborative approach. You will review and implement education and training packages, develop team capability, and lead quality improvement initiatives that promote best practice across NDIS and complex discharge processes. By fostering strong communication and collaboration within the NSR program, you will contribute to shaping a culture of excellence, compassion, and innovation in patient-centred care.
About You
You are a highly skilled and motivated allied health professional who leads with clinical expertise, empathy, and collaboration. With a strong foundation in complex discharge planning and multidisciplinary teamwork, you bring advanced problem-solving abilities, clear communication, and a commitment to improving outcomes for patients with diverse and complex health needs. You are passionate about driving innovation, supporting others’ growth, and contributing to high-quality, patient-centred care within the Neuroscience and Rehabilitation program.
Clinical Expertise & Leadership: Hold appropriate professional registration (Physiotherapy, Occupational Therapy, Psychology, Social Work, or Speech Pathology) with extensive experience in complex discharge planning, patient assessment, and evidence-based care.
Collaborative Communication: Skilled in engaging effectively with patients, families, colleagues, and community partners, providing clear guidance and advocacy while fostering teamwork and inclusion.
Strategic & Organisational Skills: Demonstrate advanced judgement, prioritisation, and initiative to manage competing clinical demands, lead service improvement initiatives, and support efficient care coordination.
Commitment to Growth & Innovation: Dedicated to mentoring staff, advancing clinical practice through research and quality improvement, and driving continuous learning across NDIS and community service pathways.
