With a strong reputation for quality, compliance, and person-centred care, we are growing fast — and we’re looking for an experienced Senior Case Manager to join our Home Care leadership team.

The Role – Senior Case Manager

This is a hands-on senior position suited to someone with a proven track record in Home Care Packages / Supported Home Care Programs. You will hold responsibility for a complex caseload, oversee compliance standards, and act as a key escalation point for the Case Management team.

Key responsibilities include:

Managing a portfolio of high-level HCP clients (Levels 1–4), including assessments, reviews, and individualised care planning

Acting as a subject-matter expert on funding, budgets, compliance and quality standards

Providing mentorship and support to junior Case Managers where required

Building strong partnerships with clients, families, and providers to deliver seamless, person-centred care

Monitoring service delivery, resolving escalations, and ensuring audit-ready documentation

Driving continuous improvement and best practice across the program

About You

We’re looking for someone who has already demonstrated success in a senior case management role and can hit the ground running.

Tertiary qualifications in nursing, social work, community services or allied health

4–5+ years’ experience in HCP / Supported Home Care case management

Proven record of managing complex clients, high-level budgets, and compliance obligations

Strong leadership capability — able to guide and mentor peers without needing a formal manager title

Excellent stakeholder engagement, problem-solving and conflict resolution skills

Passion for driving client independence, dignity and quality of life
