About the role

Bowen Flexi Care is seeking an experienced and dedicated Support Coordinator to join our team in Bowen, Queensland. This is a casual role that will involve providing Support Coordination to NDIS participants in our local community and beyond. You will play a pivotal part in helping our participants maintain their independence and quality of life.

What you'll be doing

Coordinating and monitoring the delivery of in-home and community-based support services

Liaising with participants, their families, and other healthcare professionals to ensure seamless care

Advocating for participants and their needs, and connecting them with relevant community resources

Maintaining detailed participant records and documentation in line with organisational and regulatory requirements

Contributing to the continuous improvement of our support services

What we're looking for

Relevant qualifications in community services, disability support, or a related field

Demonstrated experience in a similar support coordination or case management role

Strong communication and interpersonal skills, with the ability to build rapport with participants

Excellent organisational and problem-solving abilities

A genuine passion for supporting people with disabilities to live independently

A current driver's licence and access to a reliable vehicle
