The Opportunity

We are seeking an experienced Support Coordinator to join our growing team on a permanent part-time basis (3 days per week), based at our head office in Mooloolaba!

The Support Coordinator will:

Provide NDIS Support Coordination and Psychosocial Recovery Coaching.

Facilitate NDIS service navigation, intake and support coordination for clients living with complex mental health conditions.

Support implementation of all supports in the plan, including informal, mainstream and community, as well as funded supports.

Advocate for participants by providing evidence-based justifications for participants plan funding when attending various Stakeholder, transition and NDIS meetings.

Build the capacity of the participant to achieve greater independence to self-direct services and supports in the longer term.

Collaborating with clients to identify their strengths, goals, and aspirations.

Interpret and understand legislation and regulatory requirements that impact on service development and delivery.

Assist with NDIS reviews and audit support, including collecting data and writing reports.

Provide group workshop delivery, networking, and facilitating a peer support group with psychosocial recovery participants.

Please note: This role is for experienced Supports Coordinators. Please do not apply if you are looking for a Support Worker position. A Support Coordinator helps individuals with disabilities or specific needs access and coordinate services and supports.

We’re Looking For:

Someone who is resilient, caring and passionate about providing quality care to a diverse range of people.

Relevant tertiary qualifications or studying towards in behavioural or human services (for example, psychology, social work or similar).

1 - 2 years experience in a Support Coordination role, ideally within the NDIS, Child Protection, and/or Mental Health sectors (desirable).

An understanding of NDIS plans, support budgets, NDIS support catalogue and NDIS Pricing Arrangement.

Ability to develop and maintain good working relationships with agency staff, participants and service providers.

High level understanding and knowledge of disability and alternative care frameworks, including knowledge of the Disability Services Act 2006 / NDIS Act 2013.

Demonstrate ability to collect data, plan, prepare and complete written reports in a professional manner for plan review purposes and unscheduled reviews.

Understanding of HSQF guidelines NDIS Practice Standards.

A commitment to strength based, solution-focused and crisis intervention techniques.

Experience working in crisis situations with a trauma informed approach and understanding of various evidence-based practices.

You are eligible to work in Australia, and you hold a current QLD driver’s licence and are eligible to obtain an LCS check, blue card and yellow card.
