Joining Future Focus means becoming part of an organisation that’s adapting, innovating, and advocating nationally for the future of our profession.
About the Role
We are seeking an experienced Senior NDIS Support Coordinator to join our high-performing Perth team.
This is a hybrid position, offering a mix of remote work and office collaboration, designed to keep you connected, supported, and fulfilled.
In this role, you will:
Deliver high-quality Support Coordination to participants with complex needs.
Strengthen each participant’s ability to navigate the NDIS and broader systems — including health, housing, justice, mental health, and social services.
Provide mentorship and informal guidance to emerging Support Coordinators in your region.
Contribute to continuous improvement and innovation in service delivery.
Participate in regular clinical and administrative supervision, ensuring your practice remains reflective, ethical, and sustainable.
What You’ll Bring
To be successful in this role, you must have:
Minimum 1 year full-time experience as an NDIS Support Coordinator — this is essential. Experience in other community or case management roles will not be considered equivalent.
A minimum Diploma qualification in Community Services, Disability, Mental Health, Social Work, or a related discipline (Bachelor’s degree highly regarded).
A deep understanding of the NDIS, including how to interpret and implement participant plans.
Experience supporting participants with complex psychosocial or disability-related needs.
Strong written and verbal communication skills, with the ability to build trust, manage conflict, and produce clear documentation.
A valid Driver’s Licence, reliable vehicle, and the ability to meet all relevant probity checks (NDIS Worker Screening Check, WWCC, Police Check, etc.).
Applicants must already reside in the Perth area to be considered.
