What the Role Looks Like
You'll work from home with the flexibility to manage meetings with participants and their supports, primarily across communities south of the river.
You'll manage your own caseload and schedule - whether that's a 9-day fortnight, working within school hours, or another routine, you get to choose.
You'll support a mix of Level 2 and Level 3 NDIS participants, to ensure variety and a balanced caseload.
You'll have plenty of autonomy, but there is always someone to bounce ideas off when needed.
Your Responsibilities
Provide high-level Specialist Support Coordination to participants with complex needs.
Collaborate and meet with participants, families, providers, and stakeholders to implement and monitor NDIS plans.
Navigate complex service systems and build sustainable support networks.
Track and meet KPIs related to service delivery and participant outcomes.
Maintain accurate documentation and manage your caseload effectively.
Uphold person-centred, values-based practice at all times.
Work as part of a supportive team. We're all in this together, sharing ideas and backing each other up when needed.
What You'll Bring
Minimum 2 years' experience including Level 3 Specialist Support Coordination.
Strong ability to work autonomously while staying connected to a team.
Ability and readiness to support participants primarily located south of the river.
In-depth understanding of the NDIS framework and person-centred practice.
Excellent communication, problem-solving, and organisational skills.
Commitment to participant empowerment and ethical practice.
Relevant tertiary qualification in Social Work, Psychology, Occupational Therapy or other Allied Health disciplines, Human Services, Community Services, Disability or Developmental Education.
Desirable: Case Management experience.
Compliance Requirements
Valid Working with Children Check, Police Clearance, and NDIS Worker Screening Check.
Current driver's licence and reliable vehicle (travel reimbursement provided).
Candidates must be based in Perth and surrounding areas to attend face-to-face meetings as required.
