Support Coordinator / Recovery Coach (Psychosocial) – Perth, WA

About the Role

In this dual role as Support Coordinator and Psychosocial Recovery Coach, you will be at the forefront of supporting NDIS participants with psychosocial disabilities to achieve their goals and navigate their recovery journey. Key aspects of the role include:

Service Delivery: Provide both Support Coordination (helping participants understand and implement their NDIS plans, connect with services, and build capacity) and Recovery Coaching (working one-on-one to motivate and mentor participants in managing their mental health and progressing toward recovery goals).

Caseload Management: Maintain a sustainable caseload of approximately 20–22 clients, ensuring each participant receives dedicated, person-centred support. You will build strong, trusting relationships and adapt support as their needs evolve.

Accountability & KPIs: Manage your time effectively to meet participant needs while also achieving performance targets. We are client-first, but we also need to keep the lights on — so KPIs matter.

Quality & Improvement: Uphold the highest standards of documentation and service quality. Through our CRM and workbook system, you’ll ensure all support actions are tracked and transparent. You’ll also contribute to continuous improvement by providing feedback on how our tools and processes can further enhance outcomes for clients. This includes using our Crisis Action Planning toolkits, Mental Health Outcomes measures, and a wide range of tailored mental health workbooks.

About You – Qualifications & Experience

We are seeking an enthusiastic, compassionate, and highly skilled professional who can hit the ground running. The ideal candidate will have:

Relevant Qualifications: Minimum Certificate IV in Disability or Mental Health (or higher), with tertiary qualifications in mental health, social work, psychology, community services, or a related discipline desirable.

Sector Experience: Proven experience (ideally 2+ years) in psychosocial disability support, mental health services, NDIS support coordination, and/or recovery coaching. You understand the NDIS framework and have a strong track record of helping clients navigate support systems effectively.

NDIS Knowledge: In-depth understanding of recovery-oriented practice and how to coordinate supports under the NDIS. Experience writing NDIS documentation or using NDIS systems will be highly regarded.

Technical Proficiency: Confidence using digital tools and software in your daily work. Familiarity with CRMs such as Nightingale, Careview, or similar is an advantage. Experience with digital signing software is also highly regarded.

Excellent Communication: Outstanding interpersonal skills – able to listen deeply, communicate clearly, and build rapport with participants, families, and service providers. You are a social professional who thrives on making meaningful connections with participants and stakeholders.

Organisational Skills: Strong time management, organisation, and problem-solving abilities. You can juggle a busy caseload, meet weekly billable hour targets, and stay on top of reporting requirements without compromising service quality.

Compliance & Screening: Current NDIS Worker Screening Check, National Police Clearance, and a valid WA Driver’s Licence are essential. A reliable vehicle is required for community visits.
