About the role

We are seeking an experienced NDIS Support Coordinator to join our team at WA Remote Support Coordination in Northam, Esperance, Kalgoorlie, Geraldton, Carnarvon, Exmouth, Albany WA and surrounds. This is a full-time role providing vital support services to people living with disabilities in the local community. As the NDIS Support Coordinator, you will play a crucial part in helping our clients understand and implement their NDIS plans, ensuring they can access the services and assistance they need to live independently.

What you'll be doing

Developing and implementing NDIS support plans for clients, tailoring services to their individual needs

Coordinating the delivery of supports and services from various providers to ensure clients' needs are met

Advocating for clients to access appropriate funding and resources through the NDIS

Building strong relationships with clients, their families, and support providers

Monitoring and reviewing clients' progress, adjusting support plans as required

Maintaining accurate records and reporting on client outcomes

Providing guidance and information to clients about the NDIS and available support services

What we're looking for

Minimum 2 years' experience in a similar NDIS support coordination or case management role

Thorough understanding of the NDIS and ability to navigate the scheme on behalf of clients

Excellent communication and interpersonal skills, with the ability to build rapport with a diverse range of clients

Strong problem-solving and organisational skills, with the ability to prioritise and multitask

Empathy, patience, and a genuine passion for supporting people with disabilities

Current driver's licence and willingness to travel within the local region
