The Opportunity

The role

Apply your expertise in assessment and biopsychosocial approaches, implement practical, innovative and non-pharmacological strategies to prevent, minimise or respond to changes in behaviour related to dementia.

This role is offered as a Permanent Full-time opportunity, based out of any of our NSW offices.

Responsibilities

Understand and engage with HammondCare Mission in Action, Model of Care, Case Management Framework and practice guidelines, policies and procedures.

Provision of professional and clinical leadership for team members

Ensuring timely allocation of incoming referrals to the appropriate consultant in alignment with DSA KPI’s and Activity Work Plan.

Responsible for office environments including safety, routine maintenance and functioning equipment and ICT.

Oversees local use and maintenance of fleet vehicles including utilisation, electronic tracking and reporting.

Responsible to delegated approvals for brokerage and travel, in line with financial delegation and HammondCare Policy.

Ensure that the wellbeing and safety of team members is considered and proactively managed.

With support of Senior Team leader maintain local key stake holder relationships

Provide regular reporting to Regional Manager on all staff performance, service quality, operational performances and issues.

Recruit team members with appropriate skills for the specific roles being recruited for.

Effectively manage customer complaints or challenging situations as they arise, with early notification to the Regional Manager, and to the Head of National Programs when needed.

Work with Regional Manager to address incidents and feedback relating to the quality of the DSA Service

Improving your own knowledge and skills as part of the Advisory Care & Partnerships team.

What do we need from you?

Tertiary qualifications in Nursing/ Allied Health or health- related degree.

Demonstrated experience and knowledge in supporting older people and carers living with dementia, with particular emphasis on psycho-social approaches to case management.

Ability to manage and support a team of skilled professionals across multiple sites and locations.

Displays strong customer service ethic and attitude

Ability to work on multiple tasks and projects simultaneously.

High level communication and interpersonal skills

Strong planning and organising skills with the ability to prioritise tasks to meet deadlines.

Strong analytical skills and attention to detail.

Good customer management skills for delivery of services to HammondCare and external customers

Strong administrative and computer skills.

A current Australian driver’s licence and willingness to travel intra and interstate

Empathy and patience

Desirable:

Eligibility for registration as a Health Practitioner in Australia

Evidence of study and or qualifications in the area of dementia

Knowledge of supporting older people and carers living with dementia
