Are you passionate about empowering individuals with disability to maintain control of the supports they receive; when, where and how they receive those supports; and from whom they receive those supports?
My Place is seeking interest from committed people who are experienced in disability services to join our team in the Perth metro area. We have two positions available that both offer flexible working arrangements:
Fixed term (9-12 months) full time (38 hours)
Permanent, full time (38 hours)
About My Place
My Place is an established not-for-profit provider of individualised and flexible supports to people with disability and their families. My Place supports around 400 people with disability to live in their own homes, or remain in their family home, and become valued and contributing members of their community. My Place does not provide any group home, day centre or other congregate care services.
About the Role
As a Co-ordinator, you will work alongside people with disability, their families and allies to co-ordinate their NDIS supports in an individualised manner. Key responsibilities include:
Meeting with individuals and their families to assess their support needs and goals
Providing pre- and post-NDIS planning support to assist individuals in navigating their funding
Developing and maintaining individual support plans and ensuring they remain current
Supervising, supporting, and managing Personal Assistants, Homesharers, and Co-residents
Ensuring all services comply with My Place policies, procedures, and relevant funding requirements
Advocating for individuals and intervening when necessary to ensure their needs are met
Conducting regular check-ins and a formal annual review of direct care funding allocations
Managing administrative tasks such as payroll, funding claims, and budgeting
Identifying opportunities to enhance the quality of life of individuals through community participation and tailored support
Ensuring compliance with Workplace Health & Safety (WH&S) policies and training support staff as needed
About You
We are looking for individuals who have:
Essential
Experience in disability, community or social services, or a related field
Experience in contemporary and innovative service design for individuals, and the implementation and ongoing management of individual support arrangements including funding and budgets.
Demonstrated knowledge of the NDIS, within an individualised service approach including a clear understanding of NDIS practice standards, Quality and Safeguarding requirements and Incident and Risk management
Competent in the use of standard computer packages and information technologies.
Ability to work autonomously from a home office or in the community.
Organised, proactive, and capable of managing multiple priorities
Demonstrated contemporary attitude and values toward people with disability including promotion of, and adherence to, My Place’s values and service charter (See: www.myplace.org.au).
Desirable
Previous experience in supporting individuals to engage and manage their own supports.
Knowledge of and/or experience in planning, claiming and review processes within an NDIA environment.
Confident in supervising and mentoring support staff
Qualifications and clearances
Possess a current Drivers Licence
NDIS Worker Screening Check (NB: if a Check is not held by you, then we can provide the forms for you to obtain a Check before the role commences).
Completion of the NDIS Worker Orientation Module (NB: If not done, the module may be completed prior to the role commencing).
