Care Partner (Non-Clinical) Home Care Packages/Support at Home
Are you a self-motivated and compassionate individual with a passion for making a genuine difference in people's lives? We have an outstanding opportunity for an experienced Care Partner to join a respected non-profit home care provider in Brisbane.
About the Organisation:
Our client is an experienced Community Aged Care provider in South East Queensland, delivering quality in-home services, across multiple commonwealth funded programs, to older Australians. Due to considerable growth, they are looking for another care partner to join their experienced team.
About the Role:
You will be passionate about empowering clients to remain living independently in their own homes.
As a Care Partner, you'll play a crucial role coordinating both CHSP and HCP (SAH) clients, including onboarding new clients and providing ongoing care management, care planning and monitoring to ensure a high-quality service. You will take the lead with organising reassessments and supporting clients transitioning from CHSP to HCP/Support at Home.
With a manageable caseload, you will thrive on building strong professional relationships with both your clients and their families and the Care Workers providing their services. You'll support and mentor care workers, oversee service continuity and coordinate in-home visits.
This is a permanent, full-time Monday to Friday, office-based role located centrally in Brisbane.
About You:
Ideally, you'll bring experience in both CHSP and Home Care Packages and have some knowledge of the new Support at Home program.
You will enjoy mentoring and supporting care workers and working as part of a team, collaborating with others, sharing ideas and working towards a common goal - providing exceptional care and services to our valued clients. You will be well-supported by the organisation and the team of Care Partners, Registered Nurses and Allied Health professionals.

What You'll Bring
Experience in community aged care with demonstrated experience and knowledge of either Home Care Packages or CHSP programs with some knowledge of the Support at Home Program (training provided)
Strong organisational skills including problem solving, prioritisation and multitasking
Excellent communication and interpersonal skills with a collaborative team spirit and "can do" attitude
Proficiency in MS Office and care management databases
Current open Australian Drivers Licence with ability to drive to clients homes
Current First Aid and CPR certification
Fully vaccinated - COVID19 and current Influenza
You will also need to have full Australian working rights, and have a National Police Check (or willingness to obtain).
