Established in 1982, Alzheimer’s WA provides care, support, awareness and information to assist people living with dementia, as well as their families and carers. Our services include advocacy, education, partnerships, training, consultancy and holistic person-centred care programs, based on a philosophy of understanding the lived experiences of those living with dementia. We provide CHSP, HCP and NDIS packages and day/overnight respite in our four homes based in Perth, Mandurah and Albany.

About the Role
This is a Full-Time permanent position based in our Albany house. As an Assistant House Coordinator, you will be the backbone of our care team—providing essential administrative support that enables our Care Coordinators to focus on client care and leadership. You will also help manage client records, coordinate schedules, and ensure compliance with aged care standards. Your work will directly contribute to improving the lives of people living with dementia and their families.
Key Responsibilities
- Assist with onboarding clients and preparing accurate Agreements and Budgets.
- Maintain up-to-date records and documentation in CRM systems.
- Support risk assessments, care planning, and service coordination.
- Liaise with registered nurses to escalate clinical concerns and assist with non-clinical tasks.
- Communicate effectively with clients and Support Workers to ensure quality service delivery.
- Document incidents, restrictive practices, and progress notes.
- Contribute to continuous improvement initiatives and compliance reporting.
- Support team leadership, performance monitoring, and staff development.
About You
- Cert III, IV or Diploma in Community Services or a related discipline.
- Minimum 3 years’ experience in aged care, with dementia care highly desirable.
- Strong knowledge of CHSP, HCP, and Aged Care Standards.
- Excellent organisational, communication, and time management skills.
- Sound proficiency in Microsoft Office and CRM systems.
- A collaborative mindset and commitment to high-quality care.
- Full working rights in Australia
- National Police Clearance (within 6 months)
- Current WA Driver’s Licence
- Fully vaccinated against COVID-19 (3 doses)
