The Role:
The Aunty Grace Clinical Care Support Manager is a critical role in the provision of evidence-based, safe, effective, and coordinated clinical care services to our clients in the Aunty Grace Community.
This role is primarily a leadership and support role, with the primary purpose being to provide direction, guidance and support to Operations team members in managing the clinical care of clients. However, the Clinical Care Support Manager may also use their discretion to directly manage clinical care for clients where there is a high degree of complexity, risk and/or urgency.
When monitoring and reviewing clinical care, the Clinical Care Support Manager considers clinical care within the overall context of clients' health, function, and independence, as well as their goals, assessed needs and preferences. This includes a flexible, person-centered, and practical approach to assessment and planning that demonstrates a thorough understanding of the requirements of the Strengthened Aged Care Quality Standards and the obligations of Support at Home Registered Providers.
The Clinical Care Support Manager pro-actively reviews client care for clinical risk, as well as responding to clients who require complex clinical intervention, therefore, advanced time management and organisational skills are required.
Reporting to the Operations Manager - Care Management, The Clinical Care Support Manager also works closely with the Quality & Practice Team, particularly the Clinical Quality Manager/Clinical Lead, to ensure clinical practice is aligned with policy and process, is monitored, reviewed and reported on to continuously improve and to ensure excellent clinical care outcomes for clients.
Skills & Qualifications
Required:
Tertiary qualifications in Registered Nursing and current AHPRA registration as a Registered Nurse
Experience working in a clinical role in an Aged Care service environment complying with the current Aged Care Quality Standards or demonstrated understanding of the new Aged Care Act (2024), Strengthened Aged Care Quality Standards and the Support at Home Program
The right to work in Australia
A National Criminal History Check / Police Check (working without supervision with vulnerable people) showing no disclosable Court Outcomes or Convictions, and no greater than 3 years since the date of issue
If since turning 16, you have been a citizen or permanent resident of a country other than Australia, a signed statutory declaration stating you have not been convicted of murder or sexual assault or convicted of and sentenced to imprisonment for any other form of assault.
Vaccination status - We require proof of the initial 2 COVID Vaccinations and strongly encourage COVID boosters and an annual influenza vaccination
A current, valid Australian Driver’s License
The client is always at the centre of their care, and you take a partnership approach to their service design and care delivery
Ability to clearly articulate, document and communicate feedback and advice to team members
Ability to manage team member relationships
A professional, courteous and friendly communication style
A collaborative, problem-solving approach to working with clients, representatives, team members, workers & commercial partners, and other internal and external stakeholders
Ability to work remotely/on-road and make use of technology (smartphone, tablet, laptop) to support client care where required
A flexible and adaptable approach that embraces continuous improvement and change, and contributes to Aunty Grace’s growth
Desirable:
Experience working in clinical management, case management or care management in either Home Care Packages/Support at Home Program or Community Home Support Program.
