About the Role

As a Care Manager, you’ll manage a caseload (up to 70 members), partnering closely with Care Coordinators and Rostering & Recruitment to ensure services are implemented accurately, promptly, and in line with each member’s care plan.

Key Responsibilities

Coordinate end-to-end care planning and service delivery for your caseload.

Champion consistent use of Maree Care systems and workflows; complete tasks on time and maintain documentation standards.

Conduct quality checks across referrals, helper notes, and rostering; close the loop on overdue tasks.

Collaborate with internal teams and external providers.

Uphold Maree Care values, maintain visible leadership, and foster a supportive team culture.

About You

Experience in care/case management within aged care or community services (HCP familiarity).

Excellent communication, coordination, and documentation skills.

Systems-minded, organised, and comfortable balancing detail with people-first leadership.

Collaborative, proactive, and committed to positive member outcomes.
