We are currently seeking an Aged Care and Disability Administration Officer on a 6 months Full time basis, reporting to the Manager Aged Care and Disability.
KEY DUTIES AND RESPONSIBILITIES
Facilitate and deliver a range of services to the standard outlined in the Aged and Disability Act and CHSP / HACC PYP guidelines. Must have Mac (My Aged Care) access.
In collaboration with Manager Aged Care and Disability actively promote the Aged and Disability program within the Aboriginal community ensuring that all eligible community members have access to the Aged and Disability services provided by the Co-operative.
Coordinating with other Aged and Disability Care team members by ensuring work activities are delivered in a timely and effective manner.
Ensuring that Aged and Disability Services and DDACL's guidelines, Policies, and Standards are adhered to and that any new procedures are understood.
To complete daily service delivery data and statistics, documenting up-to-date client records.
To provide onsite assessments, including planning service provision and completing onsite Risk management assessments.
To provide support to Aboriginal community members and their carers who are receiving assistance through the Aged and Disability Care program.
To function as a team member in close liaison with other DDACL staff and to ensure that appropriate client referrals are made, as necessary, to those staff and other Aged Care providers in the region.
To assist in maintaining links with other Aboriginal and mainstream community health care services, including Aged Care Assessment Teams (RAS) .
To be familiar with the latest trends in Aboriginal health care and to obtain/disseminate information about health programs and standards.
To attend and participate in general staff and Aged care & disability planning meetings and to participate in ongoing training.
To actively promote the philosophy and need for Aboriginal specific social & community service and health services and other programs.
To act at all times in a professional manner in accordance with the policies and practices of the Co-operative and to ensure the utmost confidentiality and privacy of medical information at all times.
All other duties as directed within the scope of the employee’s skill level and training.
KEY SELECTION CRITERIA:
Essential Qualifications:
A Certificate IV in Aged Care, Leisure and Health, or a Diploma in Nursing.
A current unrestricted Victorian Driver’s License.
Valid Working with Children's Check
Understanding the NDIS referral process
Essential Criteria:
A thorough understanding of the Aged and Disability Program (CHSP / HACC-PYP ) including guidelines, policies and Quality standards and assessment procedures. Broader understanding of new changes to the new Aged care Act.
Proficient computer skills are necessary in Microsoft Office suite – Word, Excel, Outlook, Community Care based programmes and experience in Community Care Rostering Systems.
Planning skills, including the ability to assess programs and give relevant feedback to the Manager Aged Care and Disability Services.
Experience in the Aged and Disability Assessment Process and the coordination of social support groups. (Access to MAC (My Aged Care)
Highly developed interpersonal skills, high standards of personal integrity and excellent time management skills, including the ability to develop staff rosters and maintenance schedules.
Good written and verbal communication skills, including the ability to maintain appropriate records and manage time effectively and efficiently.
A demonstrated ability to work effectively as part of a small team.
A good understanding of Aboriginal health issues and culture, and knowledge of the services available to the community within our catchment.
OTHER DESIRABLE SKILLS, KNOWLEDGE AND EXPERIENCE
1. Experience and training working in a similar role.
2. A First Aid and CPR Certificate valid for the past six months, and any other relevant instructional health training
Other Relevant Criteria:
Completion of a satisfactory background and police check.
Possession of or a preparedness to obtain a “Working with Children Check” card.
A current unrestricted Victorian Driver’s licence.
Proof of full vaccination status.
