About Us
South West Community Care is a not-for-profit Organisation and Registered Charity local to the South West, providing aged care and in-home services to the local community. It is managed by a Chief Executive Officer (CEO) who is supported by a Chief Governance Officer and Management Team overseeing operations and supporting a team of over 120 employees and volunteers. The CEO, in conjunction with a volunteer Board of Management, sets the strategic direction for the organisation.
South West Community Care is managed locally and embraces the ‘Wellness’ Approach when delivering care to eligible older Australians in the Community approximately 1000 clients currently, providing services designed to maintain independence dignity and quality of life for our ageing community. These services are delivered by compassionate and caring staff working within the guidelines set out by the Aged Care Charter of Rights and Code of Conduct for Aged Care to deliver person centred care to optimise positive outcomes.

Position Purpose
Reporting to the Quality and Client Experience Manager, the purpose of your role is:
Responsible for daily monitoring and planning of Associated Providers to ensure services are aligned to client needs, care plans and approvals. The role will oversee the scheduling and financial accuracy of service delivery as well as outgoing communication and record keeping of Associated Provider interactions.
Position Responsibilities
As an Associated Providers Administration Officer, you will be responsible for and assisting with:
Associated Providers Specific
Monitoring incoming service requests and align to Associated Provider as per capacity and suitability. Allocation of ad hoc and permanent contracted services to ensure alignment with service delivery goals and objectives.
Communicate on related matters, such as planned and unplanned leave, scheduled services and changes to service availability.
Resolution of concerns raised by clients in relation to Associated Providers.
Client Specific Functions
Respond to queries from clients which are specific to scheduling matters.
Administrative Functions
Completes end of month processes for the purpose of generating monthly invoices.
Ensure all communications and interactions are accurately captured and recorded in case notes.
Monitor service delivery outputs to service delivery targets.
Assist in the preparation of reports for data collection and management reporting
Provide relief coverage for Administration where required.

- Certificate III in Business/Administration or equivalent.

Office Base Location
Based at the Australind Office but you may be required to work from other sites.
