About the role

We are currently looking for an experienced Commonwealth Home Support Program (CHSP) Manager to join our Team. Key duties will include:

  • Leading the planning and delivery of CHSP services in line with contracts and organisational goals
  • Preparing the program for near-term growth.
  • Overseeing transition readiness for the Support at Home program and a client-directed funding model
  • Embedding wellness and reablement principles across all CHSP streams
  • Consolidating and streamlining internal systems, processes, policies and procedures
  • Monitoring program and team performance with clear KPIs and accurate reporting (incl. DEX)
  • Coaching and developing team members to build a high-performing, person-centred culture
  • Ensuring compliance with CHSP Manual, Activity Work Plan, grant agreements, Aged Care Act and Strengthened Aged Care Quality Standards
  • Managing budgets, identifying risks, and ensuring sustainable service delivery
  • Representing Holdsworth with clients, carers, partners and the community

Requirements

We are seeking a candidate who:

  • Has several years’ experience leading CHSP-funded programs
  • Brings strong knowledge of CHSP guidelines, aged care reforms and regulatory requirements
  • Holds a current Driver’s Licence and is prepared to complete a Police Check and Working With Children Check (as applicable)
  • Can lead teams, drive service quality and deliver on KPIs and budgets

Desirable: Relevant tertiary qualifications in Health Care Administration, Social Work, Community Services or related field
