The Role:

Our client is seeking an experienced Manager – Coordination Services to oversee national programs across NDIS Support Coordination and Clinical Care Coordination. This is a leadership position with a strong focus on quality, collaboration, and impact. The successful candidate will manage a team of 15–20 staff, including Social Workers, Nurses, and Support Coordinators, while also holding a small caseload to remain connected to frontline service delivery.

Key responsibilities include:

Leading and coaching a multidisciplinary team to deliver high-quality, person-centred coordination services

Overseeing programs such as NDIS Support Coordination (Levels 2 & 3), hospital discharge support, GP Social Work, and complex mental health coordination

Ensuring compliance, quality, and performance across programs, including reporting and service outcomes

Supporting workforce planning, staff allocations, and recruitment oversight

Driving service innovation and contributing to research, data, and system reform

Requirements:

Qualifications: Degree in Social Work (preferred) with AASW membership, or Nursing with relevant experience. Accredited Supervisor status desirable (or willingness to work towards it).

5+ years experience working with clients with severe or complex mental health needs (flexibility for strong candidates with slightly less).

Demonstrated leadership/management experience in health, disability, or community programs.

Strong understanding of NDIS, clinical care coordination, and community-based health systems

Clinical governance, compliance, and reporting expertise

Excellent people leadership, coaching, and stakeholder engagement skills

Ability to balance operational oversight with direct client practice
