The Opportunity
We are seeking an experienced Clinical Care Coordinator to join a modern, well-established aged care provider in Western Sydney. This role is key in overseeing the day-to-day clinical operations and ensuring residents receive high-quality, person-centred care.

• Lead and support the nursing and care teams, fostering a positive and collaborative workplace culture
• Oversee clinical governance, care planning, assessments, audits, and regulatory compliance
• Monitor resident health outcomes and ensure timely and appropriate interventions
• Ensure safe and effective medication management and infection control practices
• Support staff development through mentoring, coaching, and education
• Collaborate with senior management to maintain high standards of care and drive continuous improvement

Requirements

• Current AHPRA registration as a Registered Nurse
• Minimum 3 years’ experience in aged care or a comparable clinical environment
• Demonstrated leadership, coordination, or clinical governance experience
• Strong knowledge of the Aged Care Quality Standards and accreditation processes
• Excellent communication, documentation, and interpersonal skills
• Ability to mentor, support, and guide nursing and care staff
• Passionate about delivering high-quality, resident-focused care
