Are you an experienced Team Leader who loves coaching people and lifting service quality? We're looking for a hands-on leader to guide our Support Workers, embed best-practice, and help shape a growing, participant-centred disability support service.

With an on-field team of 70+ professionals, we partner with people with disability and their families to provide practical help and foster relationships built on trust, respect and compassion.
Building on this strong foundation, we're expanding the reach of our disability supports across home and community settings, and we're seeking a Team Leader with the practical know-how to help shape a safe, quality-led service.
Role overview
You'll lead a team of Support Workers and embed best-practice disability supports across home and community environments. As the day-to-day go-to, you'll guide staff, ensure support plans are followed, maintain accurate and compliant records, and work closely with families, Support Coordinators and allied health.
You'll also contribute to process refinement, service quality and compliance uplift as the disability support stream scales.
In the early phase, you will provide targeted assistance to Bookings & Scheduling for our Home Care workforce to maintain strong service continuity while the Support Work team expands.
Key responsibilities
Leadership & Team Support
Manage and supervise a team of support workers, ensuring consistent, person-centred care.
Act as the first point of contact for day-to-day guidance.
Provide mentoring, training, and coaching to staff.
Conduct regular check-ins, team meetings, and performance discussions.
Support onboarding and competency sign-off for new starters (buddying, shadow shifts, skills checklists).
Client Care Oversight
Ensure client support plans are implemented effectively and adapt to changing needs.
Monitor quality of service delivery in line with NDIS standards.
Advocate for clients and escalate concerns where required.
Manage more complex client situations requiring leadership experience.
Coordinate the initial establishment of new participants.
Undertake spot-checks and file audits to ensure notes, risk controls and strategies are up to date.
Administration & Coordination
Assist with rostering and scheduling
Maintain accurate records (shift notes, incidents, compliance documents).
Ensure staff compliance with policies (health & safety, safeguarding, incident management).
Liaise with families, support coordinators, and allied health professionals.
Track service KPIs (roster fill %, incident close-out times, plan hours vs. delivery) and report trends.
Continuous Improvement
Identify and report service delivery gaps or risks.
Contribute to ideas that improve outcomes and efficiency.
Help embed organisational values, policies, and culture within the team.
Co-develop simple SOPs/checklists to standardise safe, quality practice (manual handling, infection control, travel).
Support readiness for internal audits and corrective actions (NDIS Practice Standards alignment).
Assist with mapping and streamlining workflows end-to-end.
Help turn business growth into operational plans.
Qualifications & checks
Cert IV in Disability, Individual Support or Community Services (or higher) preferred; strong relevant experience considered.
Demonstrated experience supervising/mentoring Support Workers.
Minimum of 3+ years experience supervising or mentoring Support Workers is required.
Solid understanding of NDIS Practice Standards, safeguarding and incident management.
Confident communicator with families, Support Coordinators and allied health.
Comfortable with rostering/scheduling systems and basic spreadsheets.
Current Driver's licence and reliable vehicle.
NDIS Worker Screening Check (or ability to obtain), current First Aid/CPR, and other role-relevant checks.
Candidates must have legal right to work in Australia with any valid work authorisation accepted.
