Team Leader /Support worker– Supported Independent Living (SIL) – Hervey Bay
Help 4 Heroes Pty Ltd, a trusted NDIS support service provider , is seeking a highly motivated and experienced Team Leader/ Support worker to oversee a complex Supported Independent Living (SIL) home in Hervey Bay. This is a full-time position leading a dedicated team supporting a participant with significant behavioural and health needs.
About the Team Leader Role
Provide day-to-day leadership, supervision, and mentoring to a team of Disability Support Workers.
Ensure staff follow all behaviour support plans, protocols, and NDIS compliance requirements.
Maintain a consistent and structured environment for the participant, including staff rosters, shift handovers, and protocols for medication, behaviours of concern, smoking, and community access.
Liaise with external stakeholders including Behaviour Support Practitioners, Support Coordinators, health professionals, and guardians.
Manage incident reporting, risk assessments, and compliance documentation.
Balance direct care with administrative responsibilities, particularly during the participant’s first 90 days in the home.
About You
You will be a confident, resilient leader with strong experience in high-complexity support. You are calm under pressure, consistent, and able to guide a team through challenging situations while keeping the participant’s safety, dignity, and wellbeing at the centre of care.
Essential:
Minimum Certificate IV in Disability or equivalent (Diploma preferred).
Demonstrated leadership/supervisory experience in SIL, residential care, or similar high-needs environments.
Strong understanding of NDIS Quality and Safeguards Commission standards.
Experience managing behaviours of concern, restrictive practices, and staff protocols.
Current First Aid & CPR, Manual Handling, Infection Control certificates.
Current Yellow Card/NDIS Worker Screening Check, Police Check, Blue Card (if required).
Current driver’s licence and reliable, insured vehicle.
Highly Regarded:
RN or background in health/allied health.
Experience leading teams through complex behavioural support plans.
Strong report writing, communication, and stakeholder engagement skills.

Who We're Looking For:

Experience or qualifications in disability support, community services, or a related field.
Exceptional interpersonal and communication skills to build strong client relationships.
A genuinely compassionate, patient, and proactive approach to supporting diverse needs.
The ability to work effectively both independently and within a team.
Strong problem-solving skills.
A valid driver's license and willingness to undergo a National Police Check.

NDIS Worker Screening Card
Valid First Aid & CPR certification
NDIS Orientation Module completion
Certificate in Individual Support (or equivalent experience)
Valid Driver's License and reliable vehicle
Manual Handling Certificate
Current Police check/certificate
