About the Role

Are you a compassionate and experienced leader ready to make a real difference?
Aligned Community Care is seeking a Team Leader – Disability Support Worker to oversee the delivery of high-quality, person-centred care to NDIS participants with complex mental health and psychosocial needs.

You’ll work closely with our Disability Service Manager to lead, mentor, and support a passionate team of Disability Support Workers — ensuring clients receive consistent, respectful, and empowering care tailored to their individual needs.

Key Responsibilities

Team Leadership & Supervision: Lead and support a team of Disability Support Workers by providing guidance, coaching, and hands-on assistance, while fostering a positive, inclusive, and high-performing team culture, and collaborating with the Area Manager to address challenges and promote team wellbeing.

Participant-Centred Care: Oversee the implementation of individualized care plans tailored to complex mental health needs, while building strong rapport with participants and supporting them in their day-to-day lives. Promote dignity, independence, and choice in every aspect of care, and address the unique needs associated with conditions such as BPD, PTSD, Schizophrenia, and other mental health challenges.

Compliance & Documentation: Maintain accurate case notes and incident reports, ensuring all documentation complies with NDIS requirements and relevant legislative standards

About You

We’re looking for a resilient, empathetic leader who thrives in complex support environments.

Experience & Skills

Minimum 1 year experience as Team leader supporting indivdiuals with mental health & complex behaviours.

Proven leadership in a Team Leader or Senior Support Worker role

Confident managing care for participants with BPD, PTSD, Intellectual Disability, etc.

Strong communication and conflict resolution skills

Excellent documentation skills

Highly organised and reliable with excellent time management

Qualifications & Requirements

Certificate III or IV in Disability, or relevant equivalent qualification

NSW Driver’s Licence + reliable vehicle

Current First Aid & CPR Certificate

NDIS Worker Screening Check

Working With Children Check

Recent (within 12 months) National Police Check

100 Points of ID

Comprehensive car insurance and vehicle registration in your name
