As a Service Coordinator for our Supported Independent Living program, you will play a vital role in supporting adults with disabilities to live independently and achieve their personal goals. This full-time position provides an opportunity to make a meaningful difference in the lives of our clients.

What you'll be doing

Coordinate the day-to-day operations of a Supported Independent Living home, ensuring a safe, comfortable, and engaging environment for residents

Develop and implement individualised support plans to help clients achieve their personal, social, and independence goals

Provide hands-on assistance and supervision to clients with daily living activities, such as personal care, household tasks, and community engagement

Liaise with families, healthcare professionals, and other stakeholders to ensure seamless and coordinated care

Manage rosters, budgets, and reporting for your assigned home

Lead, mentor, and support a team of support workers to deliver high-quality, person-centered care

What we're looking for

Relevant qualifications in Disability, Community Services, or a related field

Minimum 2 years' experience in a similar Supported Independent Living or disability coordinator role

Strong communication and interpersonal skills, with the ability to build rapport with clients and their support networks

Demonstrated problem-solving skills and a solution-focused mindset

Excellent organisational and time management abilities

Flexible and adaptable, with the ability to work as part of a team

Passionate about making a positive difference in the lives of people with disabilities

Experience in working with clients with challenging behaviors is preferred

Current Australian driver's license and reliable vehicle
