Purpose of Position
The Clinical Care Coordinator is responsible and accountable for supporting the Registered Nurses and Care staff in the clinical management of the facility, the delivery of clinical quality care, maintenance of standards and promotion of a positive work environment.
The Role: You will be working 0830-1630 every Tuesday & Wednesday + every second Monday (37.5 hours per fortnight)
Responsibilities include
Monitor the care plans of residents
Act as an adviser in response to the complex nursing and care needs of all residents
Practice continuous improvement activities to maintain a high standard of client care
Monitor the implementation of care plans and regularly review and update as appropriate
Manage each resident’s condition and progress, and ensure that documentation is regularly evaluated in accordance to SCC guidelines and accurately reflects ongoing care
Liaise as required with Doctors, Paramedics, hospitals, health care professionals and other specialist service providers
Requirements
5 years’ experience in caring for the aged or health related area
Registered Nurse experience
Strong leadership skills
Good interpersonal and written English communication skills
Ability to play an active and positive role as part of our Care Team, providing quality care and support to frail aged people
Numeracy skills – reconciliations and statistics
Sensitivity to and understanding of the needs of the elderly in a residential care setting
