We are seeking an experienced Clinical Care Coordinator to step into a vital leadership role within a welcoming aged care home. This is a hands-on contract opportunity where you’ll make an immediate impact, support the team, and ensure residents receive exceptional, person-centred care.

What you’ll be doing:

Oversee clinical governance, compliance, and accreditation requirements

Collaborate with the Facility Manager and Care Manager to meet clinical KPIs

Conduct assessments, care planning, and reviews to ensure best practice

Support staff education and development in line with aged care standards

What you’ll bring:

Current AHPRA registration as a Registered Nurse

Demonstrated experience in a Clinical Care Coordinator / Care Manager role in aged care

Strong leadership and communication skills

Solid knowledge of aged care legislation, ACFI/AN-ACC, and clinical compliance

Ability to hit the ground running and adapt quickly in a fast-paced environment
