This community role services clients across the Mandurah region and involves periodic travel to Bunbury and occasional visits to Perth.

Duties include:

• Providing safe, contemporary, evidence-based nursing care that helps clients live comfortably and safely at home
• Completing holistic clinical assessments, early detection and prevention, and health promotion
• Implementing and reviewing personalised care plans, including case conferencing and accurate documentation
• Delivering complex wound care and medication administration within scope of practice
• Coordinating care and communicating with clients, families, GPs, hospitals, and allied health professionals
• Contributing to clinical governance, quality systems, and incident reporting
• Providing guidance to ENs and PCWs on shift and escalating issues appropriately

About you

• Current AHPRA registration as a Registered Nurse
• Relevant nursing qualifications and previous experience in a similar role
• Proven skills in clinical assessment, documentation, medication management, and wound care
• Strong communication skills, a customer-first mindset, and the ability to work autonomously
• Confidence using mobile technology for care delivery and documentation
• Valid Australian working rights (e.g., Passport, Birth Certificate)
• A National Police Check (valid within the last 3 years) or willingness to obtain one
• Flu vaccination (or willingness to obtain) and evidence of COVID-19 vaccinations
• Australian Drivers licence
