At Elouera Women’s Residential Service, we provide a safe, supportive, and inclusive rehabilitation environment for women aged 18 and over. We prioritise access for women with children, Aboriginal and Torres Strait Islander women, pregnant women, and those from rural and regional communities across NSW. Children are welcome to reside with their mothers, recognising the importance of keeping families connected during recovery.
We are currently seeking a compassionate and proactive Care Coordinator to join our medium-sized, multidisciplinary team located at the tranquil residential property just outside Orange, NSW.
This newly created role offers a unique opportunity to support individuals with alcohol and other drug (AOD) challenges by ensuring their health and social needs are met alongside their treatment journey.
Your Opportunity
In this role, you will support clients from preadmission through to their stay in the program, ensuring their complex health and social needs are addressed alongside their AOD treatment. You will collaborate with the Elouera clinical team and external agencies to develop and implement initiatives that prevent health escalations and promote holistic care.
Your daily tasks will include coordinating health appointments, liaising with external providers such as GPs, dentists, nurses, and pharmacists, and advocating for clients to reduce stigma and promote empowerment. You will also provide psychoeducation and brief interventions to support clients in understanding and managing their health needs.
Your work will be instrumental in helping clients access timely and appropriate care, while also building their capacity to manage their health independently beyond the program.
This is a part-time role (19 hours per week), based on-site with some flexibility in working days. The gross hourly pay rate for this position ranges between $44.59 and $47.99 per hour plus superannuation and the option to salary package. The salary on offer will be commensurate with the successful candidate's skills, knowledge and experience.
What You’ll Bring
You don’t need to be an AOD specialist, but experience in community services, mental health, or related fields is essential. Most importantly, you bring an authentic commitment to empowerment, dignity, and holistic wellbeing for all women and their children.
You will also have the following:
- Relevant tertiary qualifications in health, social work, or related disciplines and or extensive relevant experience.
- Demonstrated experience in care coordination, service navigation, or case management.
- Experience working with individuals experiencing significant alcohol and other drug (AOD) and/or mental health issues.
- Proven ability to manage clients with complex physical and social care needs.
- Demonstrated commitment to client-centred care and holistic support.
- Strong interpersonal skills with the ability to relate positively to a diverse range of individuals.
- Demonstrated ability to work collaboratively within a multidisciplinary team environment, alongside proven capacity to work independently with minimal supervision.
- The ability to consult, liaise, and negotiate effectively with hospital and community health providers, General Practitioners, community agencies, and non-government organisations to sustain health outcomes for clients.
- Driver’s Licence and Working with Children Check or the ability to obtain this.
