• 0.6, Weds-Fri preferred, max-term until July 2026
• Based in Campbelltown, on the lands of the Dharawal People
• $55.98 per hour + 12% Super + Generous Salary Packaging benefits of up to $18,550

My Care Partners (MCP) is a collaborative, person-centred program supporting people with chronic and complex conditions to improve health outcomes, reduce preventable hospitalisations, and maintain wellbeing at home. Each is proud to deliver this program across South Western Sydney in partnership with local general practices and health services.

Your role, your impact

As a Registered Nurse Care Coordinator, you will play a vital role in improving the health and wellbeing of patients living with chronic and complex health conditions across South Western Sydney. Using your nursing expertise, you will provide coordinated, individualised care focused on empowering patients to actively manage their health and access appropriate services.

You’ll support general practices by enhancing care planning, improving communication across healthcare providers, and helping patients navigate complex systems.

You will report to the MCP Clinical Lead and work closely with general practitioners, practice nurses, practice managers, allied health professionals, and local health networks.

Key responsibilities:

• Conduct comprehensive health assessments and support patients in managing chronic conditions
• Deliver care coordination and navigation for at-risk patients enrolled in the program
• Support general practices with care planning, patient enrolment, and program delivery
• Use digital tools to enhance communication between healthcare providers
• Reduce preventable hospital visits through proactive chronic disease management
• Promote inclusive care for Aboriginal and Torres Strait Islander, CALD, and LGBTIQA+ communities

Who we’re looking for

You’re a passionate, skilled Registered Nurse with a strong understanding of chronic disease management and the community health sector. You thrive in a collaborative environment, enjoy problem-solving, and build positive relationships with patients and partners alike.

You’ll bring:

• Bachelor of Applied Science (Nursing) or equivalent
• Division 1 Registered Nurse with AHPRA registration
• RN Grade 2 Year 4 minimum experience
• Clinical experience in chronic disease management and working with complex patients
• Skills in care planning, health education, and patient-centred care
• Strong communication and stakeholder engagement skills
• Understanding of community, primary health, and the social model of health
• Experience working with diverse populations including Aboriginal and Torres Strait Islander and CALD communities

Nice-to-have:

• Experience working in general practice or multidisciplinary teams
• Familiarity with digital health tools and care coordination platforms
