ACCOUNTABILITIES
1. SUPERVISOR - NIGHT PATROL | HOUSING | T120 and AOD teams.
Regular team meetings
2. Individual / team development - skills and knowledge development
3. Overview the coordination of all program activities
4. Allocate and manage resources
5. Establish and maintain relationships with key stakeholders
6. Ensure referrals are appropriate, timely and followed up (as required)
7. Reporting - as per funding requirements
8. Data management
9. Attend stakeholder meetings and liaise with services to improve outcomes
10. Undertake home visits as required
11. Budget management
NIGHT PATROL
Accountable for the day-time-activities that will ensure the Night Patrol delivers appropriate and effective services, including but not limited to:
1. Transporting youth home or to a safe place at night;
2. Providing an additional safekeeping presence which contributes to a decrease in the general crime rate in town and creates a direct communication line with police;
3. Encouraging and engaging with the youth;
4. Providing positive role models afterhours, and;
5. Provide referrals to support services based on the varied needs of the young people
HOUSING TEAM
1. As above at Pt. 1
2. Recruit new Housing person
3. Coach / mentor replacement
4. Develop program schedules for team to follow e.g., remote visits schedule, rent assessments, VOID inspections, maintaining payment arrangements in Habitat

AOD

2. Manage 3x SBTs (School Based Trainees) - AOD Peer Support Workers
3. Support SBTs to complete Cert 11 in Community Services (CHC22015)
4. Support AOD Team Leader and/or other AOD Workers (non-SBTS) to complete Cert1V in Community Services
2. REFERRAL AGENCIES / SUPPORT SERVICES
1. Support program coordinators to network with external service providers to establish good working relationships to facilitate effective referral pathways.
2. Support program coordinators to source materials / information to develop the knowledge and skills for themselves and their team members
3. Support program coordinators to providing briefings / training to their individual teams to improve service delivery.
4. Support program coordinators to set up and run meetings etc.
3. DATA COMPLIANCE
1. Ensure all data is uploaded on time and complies with identified standards
2. Analyse the data across all the programs to identify emerging trends / needs etc., that need to be addressed - either by the engagement of stakeholders, bringing the program coordinators together to review & discuss etc.
3. Coach staff in using appropriate tools to capture data
4. Ensure that staff:
a. Maintain accurate and secure client records.
b. Ensure client privacy and confidentiality.
c. Adhere to the corporation’s Code of Conduct.
5. Review case management notes to ensure professional standards are maintained, referrals are appropriate, and progress is monitored and assessed.
6. Ensure appropriate evidence is captured to enable the completion of program reports, meeting deadlines as per the funding contract.
