About the role

We are seeking an experienced Clinical Care Co-Ordinator to join a large aged care provider, in 📍regional Victoria. This is a full time position that will play a vital role in ensuring our residents receive the highest quality of care and support. As a Clinical Care Co Ordinator, you will be responsible for overseeing the day to day clinical operations and leading your team of dedicated nursing and care staff.

Job Description 🎯

Coordinate and oversee the delivery of clinical care to our aged care residents

Manage the nursing and care staff, ensuring they are fully trained and providing excellent care

Monitor and evaluate clinical outcomes, implementing quality improvement initiatives as needed

Liaise with residents, their families, and the broader healthcare team to ensure seamless care

Maintain accurate and up-to-date clinical documentation

Ensure compliance with all relevant aged care regulations and standards

Support the development and training of the nursing and care staff

Requirements ✅

Registered Nurse with a minimum of 5 years' experience in a large aged care facility

Strong clinical skills and knowledge of aged care best practices

Excellent communication and interpersonal skills

Demonstrated leadership and management experience

Proficient in clinical documentation and data analysis

Commitment to delivering high-quality, person-centred care

Understanding of aged care regulations and accreditation standards
