Our Aged Care Services team empowers older people to live long and well at home, offering a range of supports from domestic assistance to nursing, social connection, and clinical care.
About the Role
We are seeking an experienced and compassionate Clinical Home Care Team Leader to provide both clinical expertise and leadership across our Home Care Package (HCP) and Commonwealth Home Support Programme (CHSP) services. This is a newly created role, and you work alongside another Clinical Team Leader reporting to the Manager of Home and Community Care.
In this role, you will:
- Lead and mentor Care Partners, Clinical Care Partners, and CHSP Care Coordinators.
- Oversee care planning, intake, assessments, and quality service delivery.
- Manage a team of 10-12 HCP and CHSP Case Managers.
- Ensure services meet participant needs, preferences, and budget requirements.
- Provide clinical guidance and support to staff for complex care situations.
- Ensure compliance with Aged Care Quality Standards and funding guidelines.
- Foster a collaborative, supportive, and high-performing team culture.
What We're Looking For
- Bachelor's degree in nursing with current AHPRA registration.
- Minimum 3 years' nursing experience, ideally in aged or community care.
- Knowledge of Home Care Packages and Commonwealth Support Program.
- Proven leadership skills and the ability to manage a multidisciplinary team.
- Strong understanding of Aged Care Quality Standards and Consumer Directed Care.
- Experience managing service budgets and meeting compliance requirements.
- Excellent communication, problem-solving, and relationship-building skills.
