We’re looking for a Mental Health Care Coordinator (RN or Psychologist) who is passionate about person-centred care and improving access to support. In this varied and rewarding role, you will be the key point of coordination for participants in our virtual health programs—guiding them through recovery journeys, facilitating referrals, and collaborating closely with coaches, clinicians, and providers to ensure seamless care delivery.

Key accountabilities and Responsibilities

Assists with the coordination of care across the care continuum

Referrals: Work with team to process incoming bookings, referrals, and correspondence from external providers. Liaise with the Provider Lead and Team to expedite referrals and their commencement in Valion Health Programs. Processing of participant data collection and updating of electronic medical records as required.

Work in partnership with mental health coaches and health coaches to deliver programs/ services through great communication and case conferencing;

Orients and educates participants and their carers by explaining the role of the mental health care coordinator; initiating the program and/or service;

Undertakes initial assessment, set-up, and follow-up sessions via video conferencing or telephone with program participants;

Communicates care plans and pertinent information with the primary care team as well as with other specialists or parties participating in the care of the patient/teammate.

Monitors the delivery of care by documenting care;

Confidentiality: Strictly maintain patient confidentiality

Provides technical Support – participants

Escalate care when appropriate

Caring Attitude: Convey a caring attitude by empathising with and comforting patients by anticipating and easing patients’ anxieties, answering patients’ questions, and promptly attending to distressed patients.

Communication: Communicate clearly and effectively with staff, participants, providers, and clients to ensure that problems and difficulties are avoided, resolved, or escalated to the appropriate individual.

Identifying progress toward desired care outcomes; reviewing the care plan with patients and with care providers

Maintains patient care database by entering new information as it becomes available, backing up data.

Respects participants and providers by recognising their rights, maintaining confidentiality.

Participates in regular team meetings;

Coordinate resources, including practitioner hours and packages, within budget allocations;

Undertakes business development activities, including stakeholder engagement; Coordinates patient-reported outcomes (PROs) collection

Complete program feedback with program participants and providers

Provides support for the health professional provider team

Complaints/compliments Staff must remain receptive to complaints and compliments in a similar and non-judgmental manner.

Workplace Health & Safety - you are responsible for taking reasonable steps to maintain your own health and safety, for following workplace health and safety procedures and policies, and reporting incidents immediately to the Clinical Lead, Operations Manager, or Director.

Assist the team in the administration of audits and provider credentialing processes. Aiding the team in various program implementation projects.

Implement a range of skills, including but not limited to coaching, motivational interviewing, behaviour modification, goal setting, barrier assessment, and problem-solving.

Provides advice and support to the team, including development of quality, organisation, marketing, and communication strategies;

Provides customised, evidence-based patient/carer education within the scope of the role

Assist with ACHS Accreditation requirements;

Assisting with the development of business policies and processes to provide quality care; coordinating information and care requirements with care providers, clients.

Selection Criteria

​​​​Registered Nurse or Psychology qualifications with a background in mental health services

Registration with AHPRA

Previous experience within care coordination/ case management

Awareness and understanding of health coaching/ mental health coaching

Demonstrated knowledge and experience in a similar role

Highly developed organisational skills

Knowledge of the Australian health sector, including funding models

Previous work

Demonstrate excellent problem-solving and analytical skills

Trauma-informed and recovery-oriented approach to care

High levels of IT literacy
