About the Opportunity
There are currently 2x fulltime positions available
The Elder Care Support Connector directly reports to the Practice Manager- Newman and is accountable to the Director of Clinical Services and is responsible for connecting with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness of their aged care entitlements. The overarching function of the ECSC is to ensure that support for clients is organised, sequenced and supported to enable effective access to appropriate aged care services both internal and external to PAMS. The ECSC will work closely with the ECS Coordinator. They will also help determine the level of local demand for aged care services, identify existing aged care services, any service gaps in the area and continue advocacy work with clients as required.
About You
To be considered for this position, candidates must be able to clearly demonstrate their ability to meet the below Essential Selection Criteria:
Demonstrated ability to communicate effectively and sensitively with older Aboriginal and Torres Strait Islander peoples
Excellent and strong relationships with the local Aboriginal and Torres Strait Islander Communities
Good communication and interpersonal skills, with the ability to build trusting relationships in community.
Demonstrated understanding of the principles of person-centered care and trauma-informed practice.
Experience working with older people.
Understanding of the internal ACCHO services available to clients.
Knowledge of or willingness to learn about the aged care landscape.
Ability to or willingness to learn how to enter data and reporting.
Demonstrated experience working unsupervised and as part of a team.
Prior use of patient information systems
In addition to the above, the desirable criteria are:
The person identifies as Aboriginal and/or Torres Strait Islander and is acknowledged as such by their community;
A qualification in community services and Aboriginal mental health first aid is desirable

This position offers the opportunity to work closely with remote Aboriginal communities and make a drastic impact to the health and well-being outcomes of these communities.
PAMS operates five sites, supported by a range of clinicians, community health workers and other ancillary workers. Although the successful candidates will have a substantive location (Newman) they must be prepared from time to time to work across any one of these sites should they be required.
PAMS operates in some of the most remote parts of Australia and staff are required to be proficient at driving a 4WD vehicle and willing to fly in light aircraft.
