About the Role

We are seeking a dedicated and culturally aware Aged Care Team Lead to guide and coordinate our Elder Support workforce. In this key leadership role, you’ll ensure the effective delivery of aged care navigation and support services tailored for Aboriginal Elders and their families.

You’ll lead a team of Aged Care Connectors, while building strong relationships with aged care networks and community stakeholders.

Key Responsibilities

Lead, supervise, and mentor the Elder Care team

Oversee day-to-day operations and client case delegation

Ensure culturally safe, person-centred aged care service delivery

Support My Aged Care registration, planning, and provider navigation

Produce timely reports and uphold compliance standards

Represent Moorditj Koort at forums and aged care initiatives

Identify service gaps and advocate for improved aged care access

What We’re Looking For

Essential:

Leadership or coordination experience in health, aged care, or community services

Strong knowledge of My Aged Care and care planning systems

Experience working with Aboriginal and Torres Strait Islander communities

Ability to manage and mentor teams in a culturally safe environment

Excellent interpersonal, communication, and stakeholder engagement skills

Strong organisational and problem-solving capabilities

Required Certifications

Valid WA Driver’s Licence

National Police Clearance (within 3 months)

Working with Children Check

Vulnerable People Check

COVID-19 Vaccination (as required)

Desirable: Qualifications in Aged Care, Community Services or related field; familiarity with MMex or similar client systems.
