Your Opportunity
The AOD Continuing Coordinated Care Program supports individuals aged 18 and over who have substance use disorders and complex needs, aiming to reduce drug and alcohol use and improve overall social functioning. We are seeking a compassionate, experienced, and highly motivated Case Manager to join our team with a aim to reduce harmful AOD use, promote recovery, and improve overall wellbeing and social functioning.
The Case Manager plays a vital role in delivering high-quality, person-centred, trauma-informed, and strengths-based case management to help consumers access and stay engaged with clinical supports and AOD treatment services. The role also focuses on supporting individuals to achieve their recovery goals, develop life skills, sustain housing, and build meaningful personal and community connections.
Your key responsibilities will be to:
- Respond to referrals from internal and external services, conduct assessments to determine suitability for support
- Deliver person-centred, trauma-informed case management tailored to the needs of individuals with complex substance use issues
- Assist consumers in accessing and maintaining connection with AOD treatment and support services
- Build and maintain strong, respectful relationships with consumers, families, carers, and other service providers
- Actively participate in internal meetings and external community or service networks and share knowledge of local services, referral pathways, and best practices with team members
- Prepare reports, referral letters, and goal plans, ensuring all administrative tasks are completed accurately and on time.
- Ensure compliance with case file and data entry standards, contributing to high-quality service delivery
- Provide coverage and support as needed, including cross-team collaboration and relief duties
- Contribute to the development and implementation of service improvements and program outcomes reporting
Requirements for Success
- Bachelors' degree with 2 years of experience or Diploma in community services with extensive experience in the AOD and/or mental health sector
- Demonstrated ability to manage a caseload of Consumers with complex needs and deliver specialised or therapeutic interventions
- Proven capacity to provide trauma-informed, person-centred support that promotes recovery and wellbeing
- Strong written and verbal communication skills, with well-developed administrative capabilities to manage documentation and reporting requirements.
- An understanding of combined lived experience and professional development about factors that support recovery and a proven ability to use this to positively support others on their recovery journey
- Driver's Licence
Before starting work with us, you will need to undertake a national police history check, qualifications, referee checks, and WWCC.
