Support older Australians to access the care they need—assess, connect, and make a lasting difference in Central Australia.
The Role
As an Aged Care Assessor, you will be responsible for conducting comprehensive and home support assessments for aged clients referred through My Aged Care.
You’ll be working as part of the Aged Care Unit, which incorporates the Aged Care Assessment Service, Memory Service, Aboriginal Liaison and Psychogeriatric Service.
You’ll be based in Alice Springs be required to occasionally travel to remote communities in Central Australia and the Barkly district via 4WD vehicle or light aircraft.
Aged Care Assessment Service
The Aged Care Assessment Service (ACAS) is a Commonwealth-funded program that supports older Australians to access appropriate care. The service conducts comprehensive assessments to determine eligibility for government-funded aged care services, including residential care, home care packages, and transition care.
About you
You're a dedicated aged care professional who’s passionate about person-centred care and confident working across clinical and remote community settings. To be successful you’ll have:
A professional qualification as an Occupational Therapist, Physiotherapist, Social Worker, Speech Pathologist, Nurse or in a related field with current registration where appropriate.
Post-graduate experience providing holistic, clinical assessment and intervention to aged clients.
Demonstrated experience working effectively within a multi-disciplinary team, and the ability to prioritise and manage caseloads in an efficient and timely manner.
Demonstrated high-level communication skills and computer skills, including report writing and maintenance of clinical records.
Demonstrated ability to act as a consultant to a wide range of organisations, service providers and individuals and work experience developing and facilitating health education training programs.
Current NT Driver’s C Class Licence or ability to obtain.
