ABOUT THE ROLE:
As a Clinical Care Partner, you’ll play a dual role of being responsible for the nursing and clinical aspects of our home care package service and coordinating and implementing Home Care Services with a dedicated caseload.
You’ll work closely with consumers, our service teams, support staff, and external providers to ensure seamless service delivery and exceptional care outcomes.
KEY RESPONSIBILITIES INCLUDE:
Conduct nursing assessments and clinical review of Consumers
Support the team with oversight of clinical service delivery
Perform after hours on call duties across the organisation's services 1 week out of 4
Coordinate direct supports and brokering services
Manage individual budgets and monitoring expenditures
Ensure compliance with Home Care Service Agreements
Communicate effectively with consumers and stakeholders
Maintain accurate records and reporting on program outcomes
You will always be supported by our dedicated team and will work closely with the Care Manager.
We will provide you with access to a fully maintained, home garaged, RLOA vehicle (and Fuel Card) to attend appointments and events as required.

ABOUT YOU
OUR IDEAL CANDIDATE WILL HAVE:
Bachelor of Nursing and current NMBA & AHPRA registration
Experience in Community Aged Care/Home Care Package programs
Strong understanding of aged care standards, guidelines, and reforms
Excellent assessment, problem-solving, and time management skills
Ability to work autonomously and collaboratively
Strong communication skills with a consumer-focused approach
Proficiency in consumer management systems and electronic referrals
Current Driver’s License and ability to pass probity checks
Strong administration and multitasking skills
WE OFFER:
This is a full time working from home role based in the Redlands / Logan LGA’s
Nurses Award 2020 - Registered Nurse Level 2 (Aged Care) - $90,421 to $93,998 per annum, plus super
Fully maintained, home garaged, RLOA Branded vehicle for work use

Salary packaging up to $15,900 per FBT year pro rata
Meal entertainment and venue hire annual cap of $2,650 per annum pro rata
