Your Community Home (YCH) is a small company working in the NDIS space for 4 years. We follow the true NDIS principles of choice and control for the client with them directing their life, their support, and their decisions. This means working closely with clients to develop trust and safety and learning to understand their limitations and strengths. We seek to work with clients and employees that are self-directed, have purpose, and are team players.
Due to long time staff finding different life directions, we seek to build a new team for a much-loved client. We are looking for candidates for an immediate start.
The successful candidates will enjoy stable weekly scheduling, supportive and accessible management, training and development specific to the client, and generous casual salary.

High Intensity Support Worker Position Description (but not limited to):
Support NDIS participants to engage in activities and tasks, including capacity building, in-home support, and community access within the Inner West Melbourne area
Support NDIS participants to access the community.
Monitor the safety and well-being of the participant and report any concerns to the management.
Competent in use of phone applications to complete and read documentation, such as participant progress notes, incident reports, as well as schedule and appointment notes.
Undertake relevant participant-specific and mandatory training modules as required, and being pro-active in asking for training as needed.
Actively contribute to a team environment through open communication, participation in regular team meetings, planning processes, and professional development /supervision.
Provide backup and support to other staff, as necessary, helping and supporting each other to create a trusting workplace environment.
Report any quality and safety incidents or hazards at work immediately to management.
Understanding and complying with the relevant YCH policies and procedures.
Qualifications (preferred but not required):
Cert IV Child Protection, Mental Health, Disability, Community Services, or similar or relevant tertiary qualification or experience in the field.
Current National Police Check.
Working with Children Check.
NDIS Workers Orientation.
Current Driver's license and fully comprehensive insurance.
Competencies and experience:
Experience working in the disability field.
Experience working with people with dual diagnosis (preferred).
Genuine passion for the field of work.
Strong engagement, enthusiasm, resilience, and self-care.
Experience in medication administration or willingness to be trained.
Understanding of choice and control, dignity of risk, and hold strong personal and workplace boundaries.
