Qualifications & experience
Diploma in Community Services qualification or similar
2 years Previous client service coordination experience in the health/aged care, community services sector is desireable
Current National Police Certificate
Full COVID-19 Vaccination Certificate
knowleage about NDIS program is beneficial
QLD Open Driver licence
Tasks & responsibilities
Ability to communicate in Chinese languages such as mandarin or Cantonese which allow you to communicate with clients from this cultural background
Demonstrated knowledge and experience in a similar role for coordinating consumer directed care services to clients eligible for a home care package and NDIS .
Demonstrate ability of good communication skills, creating care plan, rostering case managing, recording and reporting which related with service management.
Relationship Building between clients and care workers, ensuring seamless communication and exceptional service.
attend community event to promote services to cultual specific community
Response potential clients request about the knowleage of the program
