CALHN is inviting an experienced and motivated Senior Occupational Therapist to join the ground-breaking CO-ACT (Care of the Older Person and Community Transition) team. CO-ACT is reshaping how older adults transition from acute care and into the aged care pathway, ensuring compassionate, coordinated, and clinically excellent support along the way.

Your Role: Specialist Leadership, Meaningful Impact:
In this rewarding leadership role, you will:
- Work within a dynamic multidisciplinary team to support older adults with complex needs, including Behavioural and Psychological Symptoms of Dementia (BPSD), as they transition through the aged care pathway from hospital settings.
- Conduct comprehensive functional and cognitive assessments, and contribute to behaviour support planning, discharge planning, and care coordination.
- Use sensory modulation and environmental strategies to enhance patient experience and manage behaviours of concern.
- Deliver home visits and environmental assessments to ensure safe, supported discharges.
- Lead equipment prescription and assistive technology recommendations, helping patients regain independence and participation in daily life.
- Collaborate with families, caregivers, and external agencies, offering guidance and advocacy to ensure continuity of care and person-centred decision-making.
You’ll be a driving force behind the clinical delivery and design of services, representing Occupational Therapy in strategic planning and system improvement. Your contribution to quality improvement, research, and service development will help shape best-practice models within CALHN.
About You
You are a highly experienced Occupational Therapist with a strong foundation in geriatric care, functional assessment, and collaborative practice. You thrive in complex health environments and are passionate about making systems work better for people.
- A recognised Occupational Therapy qualification and current AHPRA registration.
- Demonstrated experience in acute, sub-acute, and/or community care settings.
- Advanced clinical reasoning and expertise in cognitive assessment, sensory approaches, discharge planning, and behavioural management.
- Confidence in mentoring junior staff and students, supporting learning, supervision, and clinical growth.
- A commitment to culturally safe, trauma-informed, and evidence-based practice.
- Strong leadership, communication, and organisational skills, with an ability to drive service improvement and innovation.
