About the role
We are seeking an experienced individual to perform the dual role of NDIS Specialist Support Coordinator and Case Manager.
You’ll work alongside people navigating significant challenges, supporting them to access the right supports and services to live a rich, meaningful life, while navigating complex systems.
Specialist Support Coordination aims to identify and reduce barriers to implementing a participant’s NDIS plan, involving collaboration with participants and their informal and formal support networks.
Case management is a holistic and collaborative process that focuses on assessing, planning, and coordinating all necessary services to meet the complex needs of clients. As a Case Manager, you will address a broader range of issues beyond the NDIS framework, enabling more creative solutions and fostering deeper client relationships.
This isn’t a box-ticking role. It’s for someone comfortable with complexity and problem-solving. The right person will value collaboration and know how to bring together services and people across various systems -working closely with stakeholders such as families, NDIS, insurers, legal representatives, medical and allied health professionals, care providers, and community services.
You’ll have the autonomy to do your best work, backed by a genuinely supportive team. Our structured 90-day onboarding process and regular monthly supervision are designed to set you up for success from the very beginning.
This role is Monday to Friday and open to full-time or part-time applicants (minimum 0.8 FTE). Office-based support is provided in the first three months to help you settle in, learn the role, and build connections with the team.
What working with us looks like
Generous remuneration ($110,000 - $120,000 per year, pro rata)
Reimbursement for all km's travelled.
Laptop and mobile phone provided
We offer a hybrid working model, with flexibility to work from home or in the office. To stay connected and maintain the positive team culture that’s helped us achieve high staff retention, we ask that all team members spend at least one day per week in our Osborne Park office. It’s a chance to catch up, share lunch, and keep building the relationships that make this team great.
We operate under the SCHADS award, offering the flexibility to work between the hours of 6am and 6pm.
We work hard, but never at the expense of boundaries or wellbeing. We actively promote a healthy work-life balance - that means no calls or emails outside of your working hours, and no laptops while you’re on leave. If you’re away, we arrange cover so you can properly switch off. You’ll get four weeks of annual leave (pro-rata for part-time), inclusive of an office shutdown over Christmas and New Year to rest, recharge, and enjoy the Summer.
Dedicated professional development budget set at $1000 per year (pro-rata for part-timers)

Who you are (Selection Criteria - Please respond to Selection Criteria in your application)
Tertiary qualified professional such as a Social Worker, Nurse, OT, Psychologist, or another allied health professional.
Current registration with AASW, AHPRA, or a relevant professional body.
Demonstrated knowledge and experience supporting people with disabilities and/or mental health diagnoses.
A working knowledge of the NDIS and relevant legislation (essential).
Strong interpersonal, written, and verbal communication skills and the ability to develop good working relationships with internal and external stakeholders.
Confidence in managing your workload in a fast-paced environment.
Demonstrated experience in complex problem-solving and the ability to be innovative.
Experience with Microsoft Office and CRM systems.
