Perth Disability Services are seeking an experienced and motivated Specialist Support Coordinator to join our professional staff on a full-time basis, helping Complex NDIS participants achieve the goals set out in their NDIS plans.

Duties & responsibilities:

Support participants with high-level needs, including those facing mental health, behavioural, or legal challenges

Create individualised support plans aligned with participant goals and update them as needs change

Connect participants with appropriate service providers and ensure delivery of high-quality, goal-focused support

Respond to crisis situations, coordinate emergency support, and ensure participant safety

Empower individuals to increase independence through coaching, mentoring, and skill development

Identify and manage risks through ongoing assessment and implementation of risk-reduction strategies

Maintain accurate records and prepare reports to meet compliance and funding body requirements

Work closely with families, guardians, service providers, and professionals to ensure coordinated care

Stay current with best practices and participate in ongoing training and supervision

Experience & attributes:

Full Australian working rights and current residence

Minimum of 3 years’ experience as an NDIS Specialist Support Coordinator

Ability to effectively work within participant budgets to deliver plan goals

Relevant industry qualifications such as Bachelor's degree in Disability and Inclusion, Social worker, Psychologist, Occupational therapist and other allied health.

Strong understanding of the complex needs and issues of people living with disabilities and their carers

Experience in needs assessment and understanding of NDIS funding instruments

Keen communication skills and computer literacy

Valid clearance checks (NDIS Clearence, WWCC)

Current driver’s licence and 100 points of ID
