Role Overview

The role of case manager is to provide high level intensive case management support to four complex client families in the Intensive Family Preservation service. Case management includes, but is not limited to, regular and frequent home visiting, teaching and modelling living and parenting skills, referral to key service providers and regular assessments and case planning.

Key Responsibilities

The key functional areas of responsibility are listed below, which may be subject to change with consultation:

Undertake case management of families referred under the Intensive Family Preservation program in accordance with the DCJ service guidelines, framework and business rules;

In collaboration with the clients, complete a comprehensive assessment of families referred to identify family needs, strengths and child safety priorities, develop and review specific action plans that will form the basis of case management and identify a clear rationale for use of brokerage funds.

Support children and families to identify and maintain connection to extended family/kin, community and culture using family finding strategies

Where appropriate, acknowledge and engage with extended family support networks to participate in casework plans to support the family and address any child safety issues.

Provide practical and hands on support for all clients, utilising brokerage funds, strategies and role modelling to assist families to develop skills that will address issues of neglect and/ or abuse.

Where indicated support parents and/or carers to develop practical skills in parenting, home and life management skills by providing advice, developing appropriate community connections and role modelling strategies.

Collect and collate data as required for purposes of evaluation and building service evidence base

Service Delivery

Support families in their home environment by means of home visiting. This should occur approximately 2 to 3 times a week for each family, unless agreed to otherwise by Program Manager.

Engage clients in the development and review of safety plans that address concerns around the safety and welfare of the child/ren.

Identify services that correspond with family needs and make supported referrals to appropriate services including other non- government and government agencies.

Provide practical assistance to the family eg assisting with home routines, role modelling play with children, driving families to appointments, cooking with families.

Reviews: undertake family review every three months and work with FACS to review risk reassessment;

Be flexible in your working hours and meet the families at a time that is most beneficial to the family. At times this maybe outside of the normally 9am to 5pm working hours.

Monitor child protection concerns, consult with Program Manager and act in accordance with the appropriate State legislation, Mandatory Reporting Guides and Barnardos Australia Safeguarding Policies and Procedures.

Maintain written and electronic records in accordance with Barnardos policies and procedures and any external requirements.

Ensure that all the financial procedures are followed, and records are maintained.

Perform any reasonable task as directed by the Program or Senior Manager that relates to the position.

Ensure all key responsibilities are conducted in a safe way and in compliance with all Barnardos Australia Policies, Procedures and other relevant internal and external obligations.

Work in an open way with families ensuing they have a full understanding of their strengths and needs and that they regularly review their goals and action plans.

Manage work priorities, personal workload and stress levels with the support of the Program Manager

Comply with the agency policies on Workplace Health and Safety in the workplace and participate as part of the organisation to provide for a safe and healthy work environment.

Conduct practice in a child focused and trauma informed manner

Essential Criteria

Qualifications/Education/Knowledge

University qualification or TAFE Diploma in social welfare or related field.

Experience

3-5 years of casework experience with intensive family support and preservation services with children and families at risk of serious harm

Demonstrated experience, knowledge and understanding of working in a manner that is culturally competent with Aboriginal and non-Aboriginal staff, community and client families.

Abilities, aptitude and skills

Demonstrated high level of written and verbal communication skills

Demonstrated high level of computer literacy and data base navigation skills

Demonstrated ability to respectfully relate to people from diverse cultural, linguistic, and socio-economic backgrounds, in a trauma informed way.

Current Drivers License

Access to a fully maintained vehicle when a pool car is not available.

Demonstrated ability to work collaboratively with colleagues, other agencies and Government authorities.

Demonstrated interpersonal skills, communication skills, time management skills, and an ability to handle crisis.

Demonstrated ability to work in partnership with families to assess, identify and achieve goals and address child safety concerns using electronic assessment and case management systems and a variety of intervention strategies.

Understanding of trauma informed practice and the impact of trauma on parenting capacity.

A sound understanding of child development.

Willingness to work flexible working hours to meet the needs of families as required.

Ability to work both independently and as a member of a multi-functional team.

Desirable Criteria

Prior experience providing support and supervision to Child and Family Support workers and a willingness to develop these skills;

Completed training and demonstrate ability to plan and facilitate parenting programs (such as Triple P) or other programs that would benefit the families involved in the Brighter Futures program.

Understanding of current NSW legislation with regard to care and protection of children and young people.

Experience in working with children and families from an Indigenous background.

Experience in the use of electronic case management tools.
