Support Coordination / Psychosocial Recovery Coaching (PRC)

Position Description:

Be a key point of contact for all NDIS Support Coordination/PRC participants.

Understand, revise and implement NDIS plans.

Proactive communication skills, including the ability to confidently build rapport and facilitate conversations over the phone.

Encourage capacity building to manage supports effectively.

Liase with stakeholders, community services, government agencies and other service providers.

Connect participants with appropriate providers.

Demonstrate excellent time management skills and be able to work autonomously.

Be able to meet deadlines to ensure participants are receiving their services in a timely manner.

Prepare and maintain case notes and reports.

Understand the frequent NDIS changes, rules and regulations. Stay up to date with any changes.

Requirements:

Hold a valid Blue card (Working with children)

Hold a valid Yellow card (Working with disabilities)

Hold a Queensland Drivers Open Licence

Hold a current National Police check

Completed NDIS Worker Orientation Module

Completed Supporting People to Stay Infection Free module

Completed First Aid & CPR Training
