About the Role

As a Support Coordinator, you’ll work closely with NDIS participants to strengthen their ability to connect with and coordinate their chosen supports. You’ll assist clients in navigating the NDIS, implementing their plans, building capacity, and ensuring their supports align with their goals and aspirations.

Key Responsibilities

Assist participants to understand and implement their NDIS plans

Build capacity and resilience through strong, respectful relationships

Coordinate and connect participants with providers and services

Monitor outcomes and ensure supports are effective and within budget

Maintain accurate case notes and documentation in line with compliance standards

Collaborate with families, service providers, and stakeholders

About You

✔ You are empathetic, organised, and passionate about person-centred care
✔ You have a strong understanding of the NDIS and experience in support coordination (or a related field)
✔ You work well independently and as part of a collaborative team
✔ You can think on your feet and respond flexibly to participant needs
✔ You have excellent communication and interpersonal skills

Qualifications & Requirements

Experience in Support Coordination or a similar case management role

Relevant tertiary qualification in Community Services, Social Work, Disability or similar (desirable)

Strong knowledge of NDIS legislation, guidelines, and local networks

NDIS Worker Screening Check

Current Police Check

Blue Card (Working with Children)

Valid Driver’s Licence and reliable vehicle
